Sirius Black was born about 1960. He was in the Gryffindor house and was 37 years old when he died in 1996.
While living in hiding, In 1995 Sirius joined the new Order of the Phoenix. He allowed them use his family house at Number 12 Grimmauld Place as the headquarters. During the battle in the Department of Mysteries, Sirius was struck by a Stupefy spell from Bellatrix Lestrange. He fell through a veil into the world of the dead.
The Stupefy spell was not fatal. It is unclear whether falling into the afterlife actually kills you or not. Sirius may well return in some form, but whether dead or alive remains to be seen.
In 2003, during an interview with J.K. Rowling she said that Sirius was "definitely dead."
